I, hereby agree to the rules, regulations, and/or provisions stipulated in Republic Act No.
10173, otherwise known as the "Data Privacy Act of 2012" Which states that:
"State protects the fundamental human right of privacy of communication while ensuring free flow
of information to promote innovation and growth. The state recognizes the vital role of information
and communications technology in nation-building and its inherent obligation to ensure that personal
information and communications systems in the government and in the private sector are secured and protected"
(Chapter 1, Sec 2)
I fully understand my rights (Chapter IV) and recognize the role of Biggs's Inc. to protect and secure the
Personal Information (Chapter V) I provided herein. Thus, I hereby give my consent to Biggs Inc. the right to keep
and use this information for and in consideration of my application for possible third party of Biggs's Inc., the authority to
contact, verify and obtain appropriate information from entites such as NBI, schools, previous
employers, character references, and others for any background check relative to
my possible employment with Biggs Inc. and hereby release such investigation. I am willing that a photocopy of this
authorization be accepted with the same authority as the original and keep all information and data of
my Employment records for filing purposes.
